Hospice team will make decisions about your personalized care plan: type, frequency of visits, and intensity of services and techniques to help patient and family.
PHYSICIAN SERVICES
Our Medical Director will coordinate with the interdisciplinary team regarding the patient's hospice care and services.
NURSING CARE
Our interdisciplinary team will provide and supervise the patient's hospice care and services. Our Registered Nurse will coordinate and implement the plan of care.
MEDICAL EQUIPMENT
Hospice will provide medical equipment that are related to patient terminal illness and related conditions.
MEDICATIONS
Hospice will provide medications that are related to patient terminal illness and related conditions.
RESPITE CARE
This type of hospice care offer relief and rest to caregivers who provide care to hospice patient.
LIGHT HOUSKEEPING
Our specially trained nursing assistants are available to provide not only light housekeeping but also bathing, general grooming, dressing and assistance with ambulation.
INTERDISCIPLINARY TEAM
Our team consist with Registered Nurse, Hospice Aide, Medical Social Worker, and Spiritual Counselor under the direction of a physician.
